Passport Form DS-3053 is a U.S. Department of State form that is used to provide a statement of consent from one parent/guardian when the other parent/guardian cannot be present for the application of a U.S. passport for a minor under the age of 16 (more information available here https://onenotary.us/ds-3053-form-passport-parental-consent/). The purpose of Form DS-3053 is to prevent international child abduction by requiring the notarized written consent of both parents or legal guardians for a minor child to obtain a passport. However, if one parent or legal guardian cannot appear in person to give their consent, the absent parent can provide a notarized Form DS-3053 that states their agreement to the passport application. Form DS-3053 is only required when both parents/guardians have legal custody of the child and when only one of them can appear in person with the child at the passport agency. It is not necessary if one parent has sole custody of the child or if the second parent has been legally declared absent or deceased.

Where can I obtain Form DS-3053?

You can obtain Form DS-3053 from the U.S. Department of State’s website or a U.S. Passport Agency or Acceptance Facility. Here are the steps to download the form from the Department of State’s website:

  • Go to the U.S. Department of State’s website.

  • Click on “U.S. Passports” at the top of the page.

  • Under the “Apply for or Renew My Passport” section, click on “Forms.”

  • Scroll down the page and look for “Form DS-3053: Statement of Consent – Issuance of a Passport to a Minor Under Age 16.”

  • Click on the link to download the form in PDF format.

  • Print out the form and fill it out completely.

How to complete DS3053 passport form?

To fill out Passport Form DS-3053, follow these steps:

  • Download the form. You can download Form DS-3053 from the U.S. Department of State’s website or obtain it from a Passport Agency or Acceptance Facility. The DS3053 PDF form has to be printed and completed using black ink. The PDF file contains the instructions. This minor passport permission form has to be filled out in its whole and written legibly to avoid any delays in the application process.

  • Provide your child’s information. Fill out your child’s name, date of birth, and place of birth in the appropriate sections of the form.

  • Provide your information. Fill out your full name, relationship to the child, address, and phone number.

  • Provide information about the other parent or legal guardian. Fill out the name, relationship to the child, address, and phone number of the other parent or legal guardian who is unable to appear in person to give consent.
